Nursing Home Care Unit, Philadelphia, PA
The Philadelphia VA Medical Center operates a 220-bed nursing home across the street from the main Medical Center building.
Mission
The mission of the VA Nursing Home Care Program is to provide compassionate care in an interdisciplinary environment to eligible veterans with sufficient functional impairment to require the level of service and skill available in VA nursing homes. Veterans with chronic stable conditions including dementia, those requiring rehabilitation or short term specialized services such as respite or intravenous therapy, or those who need comfort and care at the end of life are served in the VA Nursing Home Care Units.
Goal of Care
The goal of care in nursing home care units is to restore the resident to maximum function, prevent further decline, maximize independence, and/or provide comfort when dying. Most VA nursing home care units are well suited to providing short-term, restorative and rehabilitative care up to 100 days, and longer term care for veterans who meet eligibility criteria and/or require end of life care, prolonged active rehabilitation, are unable to sustain a placement in a community nursing home, or lack clinically appropriate community alternative.
Admission Criteria
To qualify for VA nursing home care unit care the following must be met:
a. The veteran must be medically and psychiatrically stable,
b. The primary type of service needed must be documented according to paragraph 3 of the Directive,
c. The anticipated length of stay is documented,
d. The anticipated discharge disposition from the Nursing Home Care Unit (NHCU) is documented, and
e. Priority for the NHCU is established and documented. NOTE: Special populations for whom community placement is difficult will receive special consideration.
f. The veteran must be enrolled in the VA for health care.

Driving Directions
Follow I-95 South to exit 22 to merge onto I-676 West/US 30 West/Vine Street Expressway towards central Philadelphia. Take the exit onto I-76 East. Take exit 346B off the Expressway for University Avenue, towards Greys Ferry Avenue and University Avenue. Go straight down exit ramp, staying towards the right. Follow road under railroad bridge, and stay on the road as it curves slightly around to the right. Turn right at the first light onto Civic Center Boulevard, and turn left into NHCU entrance.
From the South
Follow I-95 North past the Philadelphia International Airport to Exit 13 (To I-76 West/291-Valley Forge). Continue following signs for I-76 West/Valley Forge. Cross Platt Bridge, and at the bottom of the bridge turn left onto 26th Street (following signs for I-76 West). Merge onto I-76 West (also known as the Schuylkill Expressway) and move cautiously to the far right lane. Exit at 346B for University Ave/Civic Center. Continue straight off the ramp onto 34th Street. Follow road until the fifth traffic light, and turn right onto Civic Center Boulevard, and take first left into NHCU entrance.
From the South West
Take Baltimore Pike (Rt. 13) East through West Philadelphia and turn right at Baltimore Avenue. Turn left at light onto Woodland Avenue and right at next light onto University Avenue. Turn right onto Civic Center Boulevard (second light), and turn left into NHCU entrance.
From the East-New Jersey
Cross the Walt Whitman Bridge (I-76 West). After paying toll, follow signs for I-76 West (Schuylkill Expressway). Take Exit 346B University Ave/Civic Center. Continue straight off the ramp onto 34th Street. Follow road until the fifth traffic light, and turn right onto Civic Center Boulevard, and take first left into NHCU entrance.
From the West and North West
Take I-476 South or I-276 (PA Turnpike) East to I-76 (Schuylkill Expressway) East towards Philadelphia.
